Catastrophic Claims
If a catastrophic event or injury occurs, please contact Suzy Baker, Director of Claims, immediately at (334) 425-4218 in order to activate the catastrophic injury management team.
Catastrophic claims are any events that result in serious injuries, including but not limited to:
- Brain or head injury
- Spinal cord injury
- Paralysis of arm, leg, or trunk
- Severe burns
- Amputation of arm, hand, foot, or leg
- Temporary or total blindness
Information needed when contacting Alliance Interstate Risk Service for notice of catastrophic claim:
- Name of injured worker
- SSN or DOB of injured worker
- Cell phone number of injured worker
- Injured worker's emergency contact name & phone number
- Injured worker's marital status
- Injured worker's occupation
- Primary language spoken
- Type of injury (if known)
- Date of injury
- Brief description of event
- Name of treating facility with street address/city/state and phone number
